Tudor Manor is registered with the Care Quality Commission for the delivery of residential care e.g. Personal care, or Accommodation for persons who require personal or nursing care.]
Our company provides services for adults including:
- Older persons (65+)
- People with physical disabilities
- People living with dementia
- People who have a sensory impairment
Tudor Manor provides a service which is tailored to your needs as an individual resident. Our services include support with:
- Personal care including support with washing, dressing, bathing and continence care.
- Domestic duties including cleaning, restaurant services, laundry, shopping etc.
- Nutrition including preparation of drinks, snacks and meals.
- Medicines management.
- Maintaining physical, emotional, and mental wellbeing.
- Hobbies, interests and personal activities.
- Other support as appropriate to your requirements.
Accessing our Service
Your service may be commissioned by the local authority, you may have a Direct Payment, or you may be paying for this service independently. When your service is commissioned by the local authority or another agency we will agree with you, and them, the support we are able to provide.
- We would complete an assessment of your needs and agree a care plan to support you at Tudor Manor.
- We would be liaising with your family/NOK , health and social care professionals and GP/doctor to gather information as part of our assessment
- We are able to complete assessments over 7 days includes weekend or evening assessment visits .
The Delivery of your Service
We will work with you to carry out a holistic needs assessment including assessing any risks you may experience.
The purpose of the assessment is to identify your health, care, social needs and personal wishes so that we can provide you with the right support to meet your commissioned and agreed needs.
Our assessment will include information about:
- What is important to you
- Your history
- People who are important to you
- How you like to live your life
- Your health, illness or injury
- What help and support you need
- What you are able to do without help
- Any new skills you want to learn
- Your beliefs, choices and preferences
Risk Assessments are an essential part of good quality care planning and are balanced with your strengths/needs to ensure any risks are identified and reduced to maximise your independence. We will aim to ensure that you and our staff are safeguarded from risk and harm.
Risk assessments will be relevant to your needs and can include areas such as:
- Environment
- Moving and handling
- Medication management
- Slips/trips and falls
- Food
- Infection prevention (including COVID-19)
- Skin integrity
- Nutrition and Hydration
- Transport
- Hot surfaces
Care Planning
The care plan is written with you (and your important people) and based on all the information gathered through your assessment. It is important that the care plan:
- Is all about you,
- Identifies what is important to you,
- Details what help and support you require,
- Sets out your choices and preference on how you want to be supported, and
- Sets out your life skills to promote your independence.
The care plan will be agreed and signed by you (or a representative such as a Lasting Power of Attorney). It provides staff with the knowledge and information to deliver person-centred, effective, responsive, safe and caring support.
After completion of the assessment and care planning process we will ensure that all documentation is agreed and signed by all relevant parties, and, where applicable, a contract will be issued.
